Retention: the part of treatment that never ends

The least-advertised fact in orthodontics: results are not permanent by default. Teeth drift throughout adult life, treated or not. Retainers are what convert 18 months of treatment into a lifetime result, and retention should be discussed — and priced — before treatment starts, not on removal day.

Your retainer options

• Removable clear retainers (Essix) — invisible, inexpensive, and the modern default. Worn nightly; typically replaced every few years as they wear.
• Fixed (bonded) retainers — a thin wire behind the front teeth. Zero compliance required, excellent for lower incisors, but demands careful flossing and periodic checks.
• Hawley retainers — the traditional wire-and-acrylic design: durable, adjustable, more visible.

Our standard is Vivera retainers by Invisalign — precision clear retainers from your final scan, in a multi-set series — with a bonded lower wire added case by case, matching the retention plan to how your teeth moved.

The schedule that protects your result

Expect full-time or near-full-time wear briefly after removal, then nightly wear long-term. “Nights forever” is the honest standard answer. Skipping months of wear then forcing a tight retainer back in is the classic route to relapse and retreatment.

Our standard retention is Vivera retainers by Invisalign — best-in-class clear retainers made from your final iTero scan and supplied as a multi-set series, so they fit precisely, last longer than ordinary clear retainers, and a lost or worn set is replaced from the drawer rather than the clinic. A bonded lower wire is added case by case where the way your teeth moved calls for it.

How retention works at our practice

Your retainers are included in your treatment fee — no separate invoice on debond day. We take the retainer scan the day the braces come off, fit the retainers within the week, and see you for retention checks through the first year at no charge. For most patients we recommend a bonded wire behind the lower front teeth (the teeth most prone to drift) plus clear removable retainers for both arches at night. If a retainer is lost or cracks, call us the same day — the drift clock starts immediately — and replacements are priced modestly.

The honest schedule

Near-full-time wear for the first few months after debond, when teeth pull back hardest; then nightly, indefinitely. "Nights, forever" is the truthful answer, and we'd rather say it now than have you discover it at forty. Clear retainers wear out and are replaced every few years; a bonded wire lasts many years but needs flossing with a threader and a check at your dental cleanings. Your dentist and we both keep an eye on it.

What relapse looks like, and what we do about it

Usually the lower front teeth: a slight overlap that wasn't there, then a little more. Caught early, a snug retainer worn consistently can hold or recover tiny drift. Beyond that, minor relapse is often a short aligner case — six to twelve months, at the limited-treatment end of the fee range — and Vivera retention this time. Frequently asked questions about retainers

What relapse actually looks like, month by month

The first weeks after debond are the highest-risk window: stretched ligament fibres pull the teeth back toward where they came from, and a retainer worn near-full-time holds them while new bone consolidates. Over the first year the pull eases, and nightly wear is enough. Then the slow forces take over — the lifelong forward drift of the lower incisors, the tongue, the lips, the bite — which is why "nights, forever" is the honest answer. A patient who skips a month at year three usually gets away with it; a patient who stops at year three is usually back at year ten with overlapping lower incisors and a limited aligner case.

Caring for the retainers themselves

Clear retainers: rinse when they come out, brush gently with clear soap (not toothpaste — it scratches), soak in a retainer cleaner a few times a week, never hot water. Replace every few years as they cloud or crack; bring them to retention checks so we can see the fit. Vivera sets: rotate through the series as sets wear; keep the unused ones sealed. Bonded wires, where fitted: floss with a threader and have your dentist check the bond at every cleaning. Both: keep the case in the pocket, never the napkin, and store the previous set as a spare.

A closer look: the Vivera series

Three or four identical sets from one scan of the finished teeth. The first goes in the week of debond; the rest stay sealed. When a set clouds, cracks, or vanishes at a sleepover, the next comes out the same night — no gap in wear, no drift, no emergency appointment. It's a small design choice that fixes the commonest way retention fails, which is not refusal but the fortnight without a retainer while a replacement is made. Retention through life's changes

Wisdom teeth: they don't push the front teeth forward, despite the folk wisdom, but the years they erupt are the years retainers get forgotten. Pregnancy: hormonal changes loosen ligaments slightly; keep the retainers in. Dental work: a new crown on a front tooth needs a new retainer; tell us. Moving away: take the retainers and the records, and see a local orthodontist for checks. Decades on: retainers still, nightly, and a fresh set every few years.
